What exactly is happening in my skin when I have acne?
Ok so acne is caused by many factors which combine to give us the dreaded spots which we all hate.
To start with we can blame Testosterone, this is an androgen hormone and it is linked with acne development. Its most often thought of as a male hormone, but women produce testosterone too, although in lesser amounts.
Testosterone in acne sufferers, through a complicated chain reaction, creates di-hydrotestosterone (DHT). DHT stimulates sebaceous glands to increase in size and produce four times the amount of sebum or oil. To boot this sebum is thicker and stickier and adheres to dead skin cells which can’t exfoliate off effectively this makes a perfect environment for the condition.
The blood in a person who suffers from acne has also been shown to be slightly acidic, this greatly influences the chemical ‘taste’ of the skin and therefore how attractive you skin will be for certain bacteria. The skin has been made into a perfect environment for the acne bacteria (p.acnes). There’s lots of food thanks to the excess sebum and dead skin cells and the climate thanks to the p.h of the skin is just perfect for them. It’s just like the bacteria have moved into a resort with all you can eat!
Now this bacteria would quite happily live on your skin munching on all the food your body provides it multiplying as it goes, but our skins bacteria is balanced like a garden and if there is too many weeds (bad bacteria) the gardeners get grumpy and want to rip the weeds out.
So what happens is our white blood cells are called in to clear away the acne bacteria which has been living in the sebaceous glands, a great big battle begins between the bacteria and the white blood cells which looks like redness and inflammation on the surface if the skin. Now as the battle continues soldiers (white blood cells) die as well as bacteria and as a parting shot like kamikazes they release digestive enzymes to further clear the infection.
Now this is all good if the war is contained within the sebaceous gland (which is actually located on the side of our hair follicles) but sometimes the infection will seep outside the boundaries of the follicle which means those digestive enzymes aren’t just taking out the bacteria but also the surrounding structures of the skin like our collagen and elastin which leads to scarring. The pus that you can actually see on the head of a pimple is a combination of those dead white blood cells, dead bacteria and digestive enzymes. The body naturally wants to purge this up and out onto the surface of the skin where it can do minimal damage.
This is the basic physiology of inflammatory acne.
But why me?!
We can get acne during different phases of our life. As a teenager we get it due to puberty which kick starts our hormones into hyper drive which then has the knock on effect of the transformation of testosterone which is linked to acne.
In adult acne there is actually a genetic link which makes our body sensitive to normal testosterone which our body then changes to the de-hydrotestosterone. There is no link between adult acne and teenage acne and in fact most people will suffer from one or the other.
Some people can also develop acne due to other factors such as stress. Stress causes the body to release the hormone cortisol, an inflammatory hormone which is known to exacerbate acne.
Acne may also be seen in people whom have increased their muscle bulk. More muscle mass equals and increase in androgens which can lead to acne.
Diet can also play a huge role in the formation and inflammation of acne.
The main culprits are:
Dairy especially milk, this is because even when cow’s milk is pasteurized the hormones still remain which obviously will lead to an increase in acne.
Omega 6- We eat way too much omega6. The western diet has a ratio of 20 omega6 to 1 omega3! We should be aiming to get that ratio down to at least 4-1 or 1-1 if possible. This is because Omega 6 is denser and actually solidifies the cell membranes, it’s inflammatory and increases sebum production.
Omega 6 is found in these foods: Peanut butter and other nut butters. All cookies, cakes, commercial desserts. All commercial snacks, chips, popcorn.
Food which has a high glycaemic index. Eating refined carbohydrates (white bread, pasta, rice, flour etc) and sugar leads to a surge in insulin and an insulin-like growth factor called IGF-1. This in turn leads to an excess of androgens which can lead to acne.
So what can you do to tackle acne?
